Enter Millberry Union from Parnassus Avenue. Go past Jamba Juice on your left hand side. Go through double doors and take the first elevator (on the left) down to floor P7. Exit the elevator and proceed down the hall to the WeID Office which will be on the right-hand side.
Please note the following things prior to your appointment:
You will be required to complete a "Daily Employee Sympton Screener" prior to entering any UCSF location. Text "screen" to 83973. If your screening reveals symptoms or COVID exposure, you will be required to contact the COVID Hotline at (415) 514-7328 for further assistance.
You must wear a mask to your appointment
You must bring a physical copy of your state issued driver's license or government issued passport with you to the UCSF ID Badge appointment. |
